Lutathera Cost: US $67,044 a Dose vs Europe's List Prices

Lutathera is a radioactive drug for certain neuroendocrine tumours of the gut and pancreas.1 It is given as four infusions, eight weeks apart.2 In the US, its maker Novartis lists it at $67,044.37 per dose.2
European health systems were quoted a fraction of that. Norway's health technology assessors worked from a list price of €20,000 a dose.3 This article sets out the prices that public documents show in five countries, and why patients once crossed borders to get the treatment.
Lutathera lists at $67,044 a dose in the US. Norway's assessors were quoted €20,000 a dose. Is the same radioactive drug really priced that differently?
What a course costs on paper
| Country | Published price | What it is | Date |
|---|---|---|---|
| United States | $67,044.37 per dose | Novartis wholesale list price | Nov 2025 |
| England | £71,500 for four doses | List price in NICE appraisal, before a confidential discount | Aug 2018 |
| Ireland | €23,500 per dose, €94,000 per course | Cost to the health service, including VAT and a mandatory rebate | 2018 |
| Norway | €20,000 per dose | Distributor's list price used by assessors | 2018 |
| Sweden | 800,000 kronor per course | Drug cost in the national assessment | Jun 2018 |
| Germany | Not published in sources reviewed | Exempted from benefit assessment | Oct 2017 |
The five prices are list or official figures for the same four-dose regimen of 7.4 gigabecquerels each.24 We have not converted currencies, because the dates differ by up to seven years.
The US: a list price that keeps rising
The US Food and Drug Administration approved Lutathera for adults on 26 January 2018.5 The approval rested on a trial of 229 patients, in which those given Lutathera went longer before their disease progressed than those on a standard hormone drug alone.5 That year, North Carolina's Medicaid programme set its maximum payment at $51,300 per vial.6
By April 2024 the wholesale list price was $55,896, according to a company spokesperson quoted by Managed Healthcare Executive.7 Novartis's November 2025 product sheet gives $67,044.37 per dose.2 At four doses, the course comes to about $268,000 at list price.
By our calculation, the per-dose price rose about 31% between North Carolina's 2018 payment cap and the 2025 list price. The two figures are not the same kind of price, so the rise is a rough guide.
Europe: prices set through assessment
The EU authorised Lutathera on 26 September 2017.1 Each country then decided whether, and at what price, its health system would pay.
England's National Institute for Health and Care Excellence (NICE) recommended it in August 2018. The recommendation covers adults with progressive, well-differentiated tumours of the gut or pancreas that carry the receptor the drug targets.4 The list price was £71,500 for four doses, about £17,900 a dose.4 NICE recommended it only with a confidential discount.4
Ireland's National Centre for Pharmacoeconomics put the cost to the health service at €23,500 per vial, including VAT and a mandatory rebate.8 A course came to €94,000.8
Norway's assessors used the distributor's list price of €20,000 a dose.3 Sweden's Dental and Pharmaceutical Benefits Agency (TLV) put the drug cost at 800,000 kronor for a full course.9 Its own analysis estimated the cost of each year of healthy life gained at 800,000 to 1.4 million kronor, against high-dose hormone treatment.9 The company's estimate was 360,000 kronor.9
Germany is the gap. Its Federal Joint Committee, which assesses new drugs, exempted Lutathera in October 2017. It found that the expected spending by statutory health insurers would be negligible.10 We found no published German price.
Why patients travelled
Cross-border treatment came first. Before Norway set up its own service, it sent about 20 to 50 patients a year to Sweden or Denmark, according to the Norwegian Institute of Public Health.11
The institute then compared the two routes. It estimated treatment at home at 492,000 to 870,000 Norwegian kroner per patient a year, and treatment abroad at 612,000 to 938,000.11 On its figures, sending patients abroad cost more.
Germany's Zentralklinik Bad Berka describes its centre as an ENETS Centre of Excellence, a status awarded by the European Neuroendocrine Tumor Society.12 Its lead physician reported in 2017 that the centre had treated 1,048 patients with this type of therapy since 2004.12 Their median survival from the start of treatment was 52 months.12 That figure comes from one centre's records, not a trial.
Patients outside Europe looked for access too. A 2016 fundraiser for a Californian patient described travelling to New York for an expanded-access trial. It called this the first time the therapy was offered in the US.13 The appeal also mentioned Bad Berka by name.13
What the headline numbers leave out
List prices are not paid prices. England's discount is confidential.4 The US figure is a wholesale list price, which insurers and hospitals may not pay in full.2
The drug is not the whole bill. Each dose is given with an amino acid infusion to protect the kidneys and with anti-sickness medicine. Ireland's assessors costed those separately.8 The prices above are for the drug alone.
Eligibility is narrow. The European approval covers adults with well-differentiated tumours of the gut or pancreas that show the right receptor on a scan.1 Not every neuroendocrine tumour qualifies.
Bad Berka's price is not public. We found no published price for self-paying patients there, so we cannot compare it with the figures above.
How to use these numbers
If you live in England, Ireland, Norway or Sweden, ask your oncologist whether you meet the approved criteria. Your health system has assessed the drug and set terms for paying for it.4839
If you are in the US, ask your insurer for a written coverage decision before the first dose. The list price makes a denied claim very expensive.2
If you are offered treatment abroad, ask for a written quote that covers all four doses, the hospital stays and the scans.
