ZipRecruiter's $65.38/hr "Medical Tourism Remote" figure comes from a loosely keyword-matched category that tracks the site's broader, sales/executive-inclusive "Medical Tourism" average far more closely than its title-matched coordinator pages -- and every coordinator-adjacent remote role checked independently (bilingual case managers, care navigators, BLS medical secretaries) clusters near $22-$24/hour, not $65.

A striking pay gap has been circulating in medical-tourism careers discussions: remote "medical tourism coordinator" jobs reportedly pay nearly three times what the same role pays in person, $65.38 an hour versus $23.06 an hour, both figures sourced to the job board ZipRecruiter. If real, that would be an unusually large remote-work premium for an administrative coordination role — most remote-work pay research finds gaps in the range of a few percentage points to perhaps 10-20%, not 200%. This piece went back to ZipRecruiter's own pages, plus several independent salary sources, to find out whether that 3x figure is actually comparing the same job in two work arrangements, or something else.

It's the latter. The $65.38 figure and the $23.06 figure come from two differently constructed ZipRecruiter pages that are not measuring the same job pool, and once you look at data for coordinator-titled roles specifically — remote or not — the apparent 3x remote premium mostly disappears.

The critical row is the fourth one. When ZipRecruiter's search matches on the job title "coordinator" specifically — rather than just the free-text terms "medical tourism" and "remote" appearing somewhere near each other — the remote-coordinator range starts at $17/hour, essentially identical to the low end of the non-remote coordinator range ($17.79/hour). That is not what a clean, title-matched 3x remote premium would look like.

Where the $65/hour figure is more likely coming from

The bare "Medical Tourism" salary page — with no "coordinator" or "remote" qualifier at all — reports an average of $148,159 per year, which works out to roughly $71 an hour on a standard 2,080-hour work year.5 That figure is close enough to the $65.38/hour "Medical Tourism Remote" number that it's a reasonable inference the two pages are drawing from overlapping, mostly non-coordinator postings: the bare category's own stated range runs from $31,000 to $399,000 a year, a spread that only makes sense if it includes senior, sales, and executive-level listings alongside anything resembling a coordinator role.

Two adjacent ZipRecruiter categories support that read. "Medical Tourism Sales" (non-remote) is listed at $36,000–$120,000 a year (roughly $17–$58/hour), and "Remote Medical Tourism Sales" at $50,000–$150,000 a year (roughly $24–$72/hour).67 The remote sales range's upper bound ($72/hour) lines up closely with the "Medical Tourism Remote" page's reported range ($59.62–$71.15/hour) — commission-eligible sales roles are a plausible, and more consistent, explanation for a $65/hour "medical tourism, remote" average than a coordination job that pays $23/hour in person suddenly tripling because it's done from home.

ZipRecruiter's own Chicago-specific breakdown of "Medical Tourism Remote" pay was based on as few as one live job listing at the time it was queried — yet it's reported with the same two-decimal precision, $67.36/hour, as a stable, well-sampled average.2

That sample-size problem matters beyond Chicago. Job-board salary pages for a narrow, low-volume search term are, by the sites' own general methodology descriptions, built from a mix of live postings actually matching the query and broader statistical/third-party modeling to fill gaps where postings are thin.8 For a search as specific as "medical tourism" plus "remote," the number of live U.S. postings at any given moment is almost certainly small, which means a single unusually well-paid listing (a senior role, a commission-heavy sales job, a bilingual clinical position) can swing the reported "average" substantially — exactly the kind of instability the Chicago figure demonstrates directly.

What coordinator-adjacent roles actually pay, checked independently

If the $65/hour figure really reflected a genuine market rate for remote medical-tourism coordination work, it should show up in comparable, better-defined job categories too. It doesn't.

The U.S. Bureau of Labor Statistics — a government source with no interest in either number — reports a median annual wage of $45,930 for "medical secretaries and administrative assistants" nationally as of May 2025, which works out to about $22 an hour.9 That's the closest official occupational category to what a medical tourism coordinator actually does (scheduling, documentation, patient communication, logistics coordination), and it lands almost exactly on ZipRecruiter's own $23.06/hour non-remote coordinator figure3 — independent corroboration between a government wage survey and a commercial job board, for the lower number, not the higher one.

Two more remote, coordination-type healthcare roles confirm the pattern. ZipRecruiter's own data for "Remote Bilingual Spanish Case Manager" shows an average of $23.87/hour ($20.67–$26.20),10 and "Remote Bilingual Care Navigator" shows $23.89/hour ($20.19–$25.96).11 Both are remote, both require an additional language skill on top of care coordination, and both still cluster within a dollar or two of the $23.06/hour non-remote medical tourism coordinator figure — not anywhere close to $65.

International patient coordinator roles — a close cousin of medical tourism coordination, usually based at a hospital's international-patient office rather than an independent agency — tell a similar story with one added wrinkle worth flagging rather than smoothing over. Salary.com puts the role at $49,909 a year (about $24/hour) as of May 2026,12 while ZipRecruiter's own salary page for the same title reports $74,111 a year (about $36/hour), with a 25th–75th percentile range of $58,613–$94,925.13 Those two sources disagree by roughly 50%, most likely because Salary.com and ZipRecruiter weight employer-reported compensation surveys and live job-posting scrapes differently — worth noting explicitly rather than picking whichever number is more convenient. Even so, neither of the two international-patient-coordinator estimates comes close to $65/hour. And for the specifically remote version of that role, ZipRecruiter's Chicago data shows just $21.12/hour ($17.36–$22.79) — again in the low-$20s, not the mid-$60s.14

The one place a genuinely higher hourly-equivalent rate does show up is licensed clinical case management, which is a different occupation, not a coordinator role. Elevance Health's June 2026 posting for a "Nurse Case Mgr Sr — Bilingual" covering Maryland, Virginia, and Washington, D.C. offers $83,748–$137,586 a year (roughly $40–$66/hour) — but the posting requires an active, unrestricted RN license and at least five years of clinical experience, and the role is hybrid, with four required in-office days a week, not remote.15 That's a meaningfully different job — clinically licensed, not fully remote — and it's the only category checked here where hourly-equivalent pay approaches the $65 figure at all.

For context on where actual seniority in this field does move the number: ZipRecruiter's own data for "Medical Tourism Director" — a genuinely senior, non-coordinator title — shows an average of $232,369 a year ($198,000–$284,500), as of May 2026.16 That's a real, large jump over coordinator-level pay, but it reflects a different job title entirely, not a remote-versus-in-office split within the same coordinator role.

Why this kind of gap is easy to produce by accident

Job boards like ZipRecruiter generate salary pages automatically for an enormous number of search-term combinations — "Medical Tourism Coordinator," "Medical Tourism Remote," "Remote Medical Tourism Coordinator," "Medical Tourism Sales," and so on are each treated as their own query against the live posting database, not as consistent slices of one underlying occupational taxonomy. There's no requirement that "Medical Tourism Remote" and "Medical Tourism Coordinator" draw from the same underlying set of job postings, and for a niche enough search term, they typically won't: one is anchored on the word "coordinator" appearing in the title, the other on "remote" appearing anywhere in the listing (location field, work-arrangement tag, or free text), with no guarantee the same postings satisfy both. When a data-and-comparison-heavy story starts from a page title alone — "$65/hour vs. $23/hour" — without checking whether the two pages are actually sampling the same job pool, a methodology artifact can look exactly like a substantive finding about remote-work pay.

What the headline numbers leave out

  • No official occupational category exists for "medical tourism coordinator." The BLS comparison used here (medical secretaries and administrative assistants) is the closest reasonable proxy, not an exact match — a coordinator role specifically involving international travel logistics and cross-border hospital liaison work could plausibly command somewhat more than a domestic medical secretary, just not on the order of 3x.
  • Job-board "averages" for thin categories are volatile by construction. The Chicago example above — an average built from as few as one listing — isn't necessarily unusual for a search this narrow; it's a structural feature of how these pages work, and it applies to the national figures too, just with a larger (still likely modest) sample.
  • "Remote" is doing ambiguous work in the $65/hour search. ZipRecruiter's query appears to match "remote" as a keyword rather than filtering to confirmed remote-only listings, which is how a hybrid role, a location-flexible sales role, or even a misclassified posting could end up counted.
  • The Salary.com/ZipRecruiter disagreement on international patient coordinator pay (roughly $24 vs. $36/hour) shows these platforms don't always agree even on non-remote, better-defined roles — a reminder that any single job-board figure, quoted alone, is an estimate with real uncertainty around it, not a precise market rate.

How to actually use pay figures like these

Anyone using this kind of job-board data to negotiate pay, plan a career move into medical tourism coordination, or write about the field should do three things before repeating a headline number: check whether the comparison is built from title-matched pages on both sides (not a loose keyword match on one side and a title match on the other); look for the sample size or listing count where the page shows one, and treat single-digit-listing averages as illustrative rather than precise; and cross-check against at least one source outside the job board itself — a BLS occupational category, a second commercial salary site, or actual live job postings — the way the medical secretaries and bilingual care-navigator figures corroborate each other here, independent of ZipRecruiter's own "Medical Tourism Remote" page.
