---

After a large weight loss, the skin often stays. Two operations deal with it. A panniculectomy removes the apron of skin and fat hanging over the lower belly. A tummy tuck, or abdominoplasty, does that and also tightens the stomach muscles and reshapes the waist.

The difference decides who pays. Insurers and the NHS treat a panniculectomy as a medical operation when strict conditions are met, and a tummy tuck as cosmetic.[^1][^3][^4] People turned down at home are the market for the tummy-tuck packages advertised from Turkey, at €3,000-6,500.[^7]

## Who pays for which operation

| | Panniculectomy (apronectomy) | Tummy tuck (abdominoplasty) | Source |
|---|---|---|---|
| US self-pay | about $5,000-7,000, front of body | ASPS average surgeon fee $8,174 | [^1][^2] |
| US insurer (Aetna) | covered if all criteria met | cosmetic | [^3] |
| NHS, Cheshire and Merseyside | funded if all criteria met | funded on the same criteria | [^4] |
| NHS, Humber and North Yorkshire | funded at BMI below 40 if criteria met | funded at BMI below 30 if criteria met | [^6] |
| NHS, Somerset | not routinely funded | not routinely funded | [^5] |
| Turkey, Acıbadem guide range | quoted case by case | €3,900-6,500 | [^7] |

## What it costs to pay yourself in the US

Healthline, citing surgeons, puts a self-paid panniculectomy across the front of the body at about $5,000 to $7,000. A version covering the whole torso runs about $10,000 to $20,000. Those figures may exclude anaesthesia and hospital care.[^1]

The American Society of Plastic Surgeons (ASPS) gives an average tummy tuck surgeon fee of $8,174. It too excludes anaesthesia and the operating room.[^2] Healthline says tummy tucks are usually not covered by insurance.[^1]

## What a US insurer requires

Aetna, a large US insurer, sets out its rules in clinical policy bulletin 0211. It covers a panniculectomy only when every condition is documented.[^3]

The apron must hang below the pubic bone, shown in photographs. The skin underneath must have a chronic rash or infection that keeps coming back over three months despite treatment. Weight must have been stable for three months. After weight-loss surgery, at least 12 months must have passed since the weight plateaued.[^3]

Aetna lists abdominoplasty among cosmetic procedures.[^3]

## What the NHS requires depends on where you live

In England, local health boards called integrated care boards (ICBs) set their own rules. [Excess skin removal on the NHS](/articles/excess-skin-removal-nhs) compares more of them, area by area. Three show how far apart they are.

**Somerset.** Its March 2025 policy says abdominoplasty and apronectomy are "not routinely commissioned".[^5] Individual funding requests remain possible.

**Cheshire and Merseyside.** Its March 2024 policy funds either operation only if all its criteria are met. Your body mass index (BMI) must be under 30 and stable for at least 12 months, after a previous BMI above 40, or 35 with other conditions. You must not smoke, and the skin folds must be infected or broken down despite treatment.[^4]

**Humber and North Yorkshire.** Its October 2024 policy is the most open of the three for an apron removal alone. It accepts a current BMI under 40 if you have lost at least 75% of your excess weight, held stable within 10 kg for 12 months.[^6] A full abdominoplasty there still needs a BMI under 30.[^6]

Somerset does not routinely fund apron removal at all. Humber and North Yorkshire funds it at a BMI under 40 for people who have lost three-quarters of their excess weight.[^5][^6]

## The Turkish alternative

Acıbadem, a Turkish hospital group, puts a full tummy tuck at €3,000-5,000 as a Turkish market average and €3,900-6,500 in its own guide range.[^7] For comparison it quotes £6,500-10,000 in the UK and $8,000-15,000 in the US.[^7] Those UK and US figures come from a seller and should be read that way.

It does not publish a panniculectomy price. It says quotes are made case by case after examination.[^7] It notes that a tummy tuck with muscle repair is "almost always classed as cosmetic", so insurers rarely fund it.[^7]

## What the headline numbers leave out

A panniculectomy turned down at home and a tummy tuck bought abroad are not the same operation. The tummy tuck is larger, with more contouring. If a complication follows surgery abroad, treatment at home falls back on the NHS or your insurer.

Acıbadem itself lists costs that quotes often leave out: time off work, help at home, and one to two weeks near the hospital before flying.[^7] None of those appear in the €3,900 figure.

The NHS policies here are three boards' rules, picked to show the range. They are not a survey, and each can change at review.[^4][^5][^6]

## How to use these numbers

If you have an overhanging apron after weight loss, find your own ICB's policy on abdominoplasty and apronectomy, or your insurer's panniculectomy criteria. Write down the BMI, the stable-weight period and the skin problems it requires, and ask your GP (family doctor) to document each one.

If you are refused, ask whether an individual funding request is possible before pricing a Turkish tummy tuck. And if you do travel, compare the full cost, including the weeks of recovery away from home.

Hanging skin after weight loss: the NHS may pay to remove it, or a Turkish clinic will for about €3,900. Which applies to you?

Insurers and the NHS split the operation in two. Removing the apron can be medical. A tummy tuck, with muscle tightening, is cosmetic.

NHS rules depend on your postcode. Somerset doesn't routinely fund it. Cheshire and Merseyside needs a BMI under 30, stable for a year. Humber and North Yorkshire accepts under 40 for apron removal.

A US insurer, Aetna, requires an apron below the pubic bone and a skin rash that has lasted three months despite treatment. If you don't meet that, Turkey sells the alternative.

A Turkish tummy tuck is a bigger operation than an NHS apron removal. The price leaves out weeks of recovery away from home, and three policies are not a survey.

Check your own board's criteria first, and ask your GP to document them. Price the trip only if you're refused.
