Wegovy is made by Novo Nordisk, a Danish company. Japan's public health insurance pays for it, with patients covering about 30% of the price. Denmark's public system does not pay for it at all, and Germany's is barred by law from doing so.

The same drug gets opposite answers because each country asks a different question. Japan asks whether a patient has obesity plus related disease. Denmark asks whether the price matches the benefit. Germany's law asks whether the medicine is for weight loss. England's answer sits in the middle, with conditions. This article compares the four.

Japan's public insurance pays most of the cost of Wegovy. Denmark, where it is made, pays none. Why would the same drug get opposite answers?

It is tempting to read this as one country being generous and another mean. But each system applies a different test, and the tests explain the answers.

Japan covers it for obesity with related illness: a body mass index (BMI) of 35, or 27 with two conditions such as high blood pressure. Patients pay about 30%, roughly ¥7,500 to ¥12,900 a month.

Denmark's reimbursement committee said in 2022 that the price did not match the treatment's value, partly because weight returns when treatment stops. It refused again in 2025 for a narrow group of teenagers.

Germany's answer is not about value at all. A 2004 law excludes medicines used for weight loss from statutory insurance, and in 2024 the joint committee confirmed Wegovy falls under it.

What this cannot tell you: whether any one country decided correctly. The decisions weigh price, evidence and law differently, and the drug's price differs between them too.

If you are relying on public cover, check your own country's test, not the drug's reputation. In England it means a specialist service and a time limit. Save this before you ask your doctor.

## Japan: covered as treatment for disease

Wegovy was added to Japan's national health insurance price list in November 2023.[^1] Coverage is limited to people with high blood pressure, high cholesterol or type 2 diabetes who have not done well enough with diet and exercise.[^1] They need a BMI of 35 or more, or 27 or more with at least two obesity-related conditions.[^1] A Japan-based guide gives a lower figure, a BMI of 30, which may describe the drug's approved use rather than the insurance rule.[^2]

Japanese patients usually pay 30% of the cost. A Japan-based guide puts that at about ¥7,504 a month at the starter dose and about ¥12,900 at the highest dose.[^2] It also says treatment must take place at certified clinics and hospitals.[^2]

## Denmark: the price did not justify it

Denmark's reimbursement committee reviewed Novo Nordisk's application in January 2022. It recommended that Wegovy should not receive general reimbursement for adults.[^4] It accepted that the drug can cut weight by up to 20%. But it judged that the price "does not stand in a reasonable relation" to its treatment value.[^4]

The committee gave a specific reason. Trials lasted at most two years, and they pointed to patients regaining weight once treatment stopped.[^4] That implies long-term treatment, at long-term cost.

The company tried again, more narrowly. In 2025 it applied for reimbursement for teenagers aged 12 to 18 with severe obesity and a cognitive or developmental disorder.[^5] The committee again recommended against it.[^5]

Private insurance followed. Sygeforsikringen Danmark, a member-owned insurer covering about 2.7 million people, stopped subsidising weight-loss medicines from January 2024.[^6] It made an exception only for members who have individual approval from the Danish Medicines Agency.[^6]

## Germany: excluded by law, not by evaluation

Germany never weighed Wegovy's value for this purpose. Its federal joint committee of doctors, insurers and hospitals confirmed in March 2024 that Wegovy is a "lifestyle medicine" excluded from statutory insurance.[^3]

The exclusion comes from the law. Medicines "used for losing weight" were excluded as a statutory benefit in 2004.[^3] The committee said it had no discretion to make exceptions, even for people with a BMI over 30.[^3]

## England: covered, within limits

England's health technology body, NICE, recommends semaglutide for weight management only within a specialist weight management service.[^7] It is limited to a maximum of two years.[^7] Patients need at least one weight-related condition and a BMI of 35 or more, or 30 to 34.9 if they meet the referral criteria for specialist care.[^7]

England also covers tirzepatide, sold as Mounjaro, for people with a BMI of 35 and at least one weight-related condition. NICE allows it to be introduced in phases.[^8] Both appraisals use lower thresholds for people from some ethnic backgrounds.[^7][^8]

## What the comparison leaves out

The decisions are not on the same price. Each country negotiates or sets its own, and NICE's recommendation depends on a confidential commercial arrangement.[^7] Denmark's committee judged value at the price it was offered.[^4]

Coverage on paper is not access. England's route requires a place in a specialist service.[^7] Japan's requires a certified clinic.[^2] Waiting times for either are not covered by the sources reviewed here.

Rules move. Denmark's 2022 recommendation was for adults and its 2025 one for a narrow teenage group.[^4][^5] Any of these positions could change with a new application or a new price.

## How to use this

Check your own country's test before assuming cover exists or does not. In England, ask your family doctor whether you meet the criteria for referral to a specialist weight management service.[^7]

If you pay privately, compare total cost over the treatment period, not a month's price. Denmark's committee refused partly because benefits fade after stopping.[^4] That applies to a private buyer too.
