Large weight loss, whether after surgery or weight-loss medicines, can leave folds of loose skin. On the abdomen this can hang as an "apron" that rubs, gets infected and makes washing hard. Removing it is surgery, and someone has to pay.

In England there is no single national rule. Each local NHS body sets its own criteria, and the ones reviewed here differ on the body mass index (BMI) limit and on how long weight must be stable. In Germany, the federal social court ruled in August 2025 that covering weight-loss surgery does not oblige an insurer to pay for the skin removal afterwards. This article compares those rules with the price of going private abroad.

Lost the weight, kept the skin. Will the NHS remove it, or are you heading for a clinic abroad?

The NHS does fund some skin removal. But each area writes its own rules, and three NHS bodies reviewed here set three different bars.

One trust excludes anyone with a BMI above 28. Another area accepts a BMI of 30 with three months of stable weight. A Manchester hospital asks for two stable years.

The counterargument: these rules do fund surgery. Each one covers removal of an overhanging apron that causes repeated skin infections or problems with daily life. Appearance alone is what they exclude.

What the rules cannot tell you: your chances. They set minimum criteria, and approval still runs through a local funding panel case by case.

Find your own area's written policy before you price a tummy tuck abroad. You may qualify for the medical version at home. Save this for your next family doctor (GP) appointment.

## Three NHS bodies, three sets of criteria

| Area or hospital | BMI limit | Weight stable for | What is funded |
|---|---|---|---|
| South Tees Hospitals NHS Foundation Trust | Excluded above 28[^1] | More than 6 months[^1] | Overhanging skin with intractable skin infection, stoma problems, or hernia repair[^1] |
| NHS Cheshire and Merseyside | 30 or less[^2] | At least 3 months[^2] | Apron removal with functional impairment; not cosmetic abdominoplasty[^2] |
| Manchester University NHS Foundation Trust, Wythenshawe | Not stated on the page | Two years[^3] | Several body contouring operations, subject to local funding approval[^3] |

## How the NHS rules work

South Tees publishes the strictest BMI limit of the three. Anyone with a BMI above 28 is excluded from all body contouring, and weight loss must have been sustained for more than six months.[^1] It funds removal of overhanging abdominal skin with "intractable intertrigo", a stubborn skin infection in the fold, or with poorly fitting stoma bags.[^1] It excludes surgery to improve appearance, including loose skin after pregnancy.[^1]

Cheshire and Merseyside, in a policy issued in March 2024, accepts a BMI of 30 or less with weight stable for at least three months.[^2] It funds apron removal where there is documented functional impairment. It does not fund abdominoplasty for cosmetic reasons.[^2] Patients outside the criteria can apply for individual funding.[^2]

Wythenshawe Hospital in Manchester runs a specialist service. It asks for weight "to have been stable for two years", and says funding approval must come from the local funding panel before referral.[^3] Its surgeons do not make funding decisions.[^3]

The difference matters. The same patient, with a BMI of 29 and weight stable for four months, would be excluded by South Tees and could qualify in Cheshire and Merseyside.[^1][^2]

## Germany: no automatic follow-on cover

German statutory insurers can pay for apron removal, but only on medical grounds. A German plastic surgery clinic describes cover as "highly likely" with a pronounced apron after massive weight loss plus skin inflammation, or restrictions on mobility or hygiene.[^6] The insurer's medical service assesses each case, and the application must be made before treatment.[^6]

The federal social court drew the line in August 2025. A woman whose insurer had paid for gastric reduction surgery asked it to pay for skin tightening and breast enlargement afterwards.[^5] The court ruled that paying for the first operation "does not automatically" oblige the insurer to pay for the second.[^4] It found her procedures were cosmetic, despite her psychological distress, and she was left with a bill of €4,284.50.[^5]

## What going private abroad costs

Turkish hospitals advertise the cosmetic version. One Istanbul hospital group quotes €3,900 to €6,500 for a full tummy tuck. A lower body lift after weight loss runs €7,150 to €14,300.[^7] It says the packages include anaesthesia, hospital nights and follow-up, and that patients should plan seven to ten nights in Istanbul.[^7]

Those prices are for an operation the NHS bodies above would not fund in any case: abdominoplasty for appearance. Apron removal for medical reasons, where a patient qualifies, costs the patient nothing at home.

## What the rules leave out

Criteria are a floor, not a promise. Every NHS policy reviewed here routes the decision through a local funding panel.[^2][^3] Meeting the BMI and stability rules gets a case considered. It does not guarantee surgery.

The policies also change. Cheshire and Merseyside's dates from 2024 and South Tees's page was last updated in 2022.[^1][^2] A rule found online may not be the one your area applies today.

Going abroad shifts the aftercare. A body lift is a large operation with a long recovery. A complication after returning home lands with the NHS or with you.

## How to use the rules

Ask your GP for your area's written body contouring policy, by name. Check your BMI and how long your weight has been stable against it.

If you have recurring skin infections under the fold, make sure they are recorded in your notes. That record is what each of these policies looks for.[^1][^2]

If you do not qualify and choose to pay, compare like with like. Ask whether a quote is for apron removal or a full tummy tuck, and what follow-up it includes once you are home.
